Pharmacists and PSS Members in Alexandra Hospital helped conduct medication review and reconciliation that enabled patients and their family to

˙Better observe drug adherence;

˙avoid duplicated medication and polypharmacy;

˙prevent unnecessary wastages and costs;

˙ensure optimal therapeutic outcomes.

This campaign was part of the patient safety initiatives and attracted very positive media attention.

PSS Vice President and Alexandra Hospital Chief Pharmacist, Ms. Fatimah MK went on TV to explain the programme and cited examples of patients who were able to save hundreds of dollars every month through such cognitive service provided by pharmacists.
