INSTRUCTIONS TO APPLICANTS FROM THE WESTERN PACIFIC REGION

A.Please open the FIP Congress Travel Grant Form and read the instructions therein carefully.

B. Please take note of the following when completing the form

DEADLINE FOR SUBMISSION – 31 JANUARY 2012

Western Pacific Pharmaceutical Forum- FIP Congress Travel Grant For Developing Countries Application is Closed

 

INSTRUCTIONS TO APPLICANTS FROM THE WESTERN PACIFIC REGION

1) Please open the FIP Congress Travel Grant Form and read the instructions therein carefully.

2) Please take note of the following when completing the form:

(a) The Total Budget should include

  1. i The fares for air & land travel (provide all flight details)
  2. ii. The cost for accommodation (provide hotel name and number of night stay)
  3. iii. The registration fees

(b) The Objectives to be achieved should include:

  1. i. The main purpose of attending the FIP Congress
  2. ii. The main objectives to be achieved (Please indicate any special interest
  3. group activities or particular sessions that you may wish to attend)
  4. iii. The after-congress plan(s) that you may have upon returning to your
  5. country after attending the congress.

(c) The Travel Plan should provide details of your travel itinerary.
(d) The Curriculum Vitae should highlight:

  1. i. Your work experience and achievements.
  2. ii. Extra activities that you may have been involved in as a pharmacist (eg.
  3. active member of the National Pharmaceutical Association, member of
  4. important committee etc.)

(e) The letter(s) of recommendation should highlight the reason why you are a deserving candidate  

      for receiving this travel grant.
(f) If you are presenting a poster presentation, please insert the abstract which you have submitted.

(g) Please include a recent photo of yourself.

3) Complete the form and save the document in the folder provided and rename the folder by indicating your family name and initials followed by an underscore and the country you reside in (eg. Chan, SJ_Malaysia; Ngu VH_Vietnam). Send the folder as an attachment by email, with a cover note, to the President of WPPF and the Chairman of the Selection Committee:

4) Candidates are nominated by the WPPF based on the following criteria.
(a) Accomplishments as a pharmacist
(b) Leadership qualities
(c) Impact on pharmacy profession in own country
